病例分享|創(chuàng)傷后鎖骨骨溶解個例報道

一、病例資料
患者37歲，女性，2018年12月12日車禍致左鎖骨及左肩胛骨折，在三門峽市中心醫(yī)院行左鎖骨及左肩骨骨折手術治療，術后半年復查X線片提示左鎖骨遠端骨溶解，術后兩年在三門峽市中心醫(yī)院行內固定取出并取病理檢查。(附隨診相關影像學資料及病理資料)

二、討論
創(chuàng)傷后鎖骨骨溶解（post-tramautic osteolysis of the distal clavicle）是一種罕見病，通過知網(wǎng)萬方醫(yī)學等中文數(shù)據(jù)庫檢索，僅有陶惠民[1]、邱乾德[2]等少數(shù)學者撰寫的相關報道；檢索外文文庫Pubmed，相關報道相對較多。創(chuàng)傷后鎖骨骨溶解報道在1936[3]年，至20世紀末，據(jù)文獻記載相關報道僅100例[4]左右。
創(chuàng)傷后鎖骨骨溶解，因為其罕見性，到目前為止其確切的病理發(fā)病機制尚不明確[5]；但回顧歷史文獻記載，其好發(fā)于兩者人群。
第一，急性創(chuàng)傷人群，1936[3]年報道的鎖骨骨溶解，其認為這種疾病的發(fā)生僅僅與肩鎖關節(jié)脫位或半脫位有有關，本文記述的個例出現(xiàn)肩鎖關節(jié)脫位,后來的文獻報道肩鎖關節(jié)脫位的患者中發(fā)生創(chuàng)傷后鎖骨骨溶解的概率為1%-21%[6-9]；隨后的報道中，鎖骨遠端骨溶解好發(fā)的第二種人群逐步報道出來，以高強度上肢運動為主的人群中，1959年報道在一位沒有外傷史的便攜式氣鉆操作員身上，認為肩部反復的局部應力是導致鎖骨遠端溶解的原因[10-12]；隨后，大量上肢為主的體育競技運動員出現(xiàn)該現(xiàn)象，包括手球、棒球、舉重等競技運動[5]?？傊壳肮J急性創(chuàng)傷和反復的局部應力是導致鎖骨遠端骨溶解的兩種原因，病理機制尚待研究。
創(chuàng)傷后鎖骨骨溶解其疾病特點在骨溶解階段是多樣的，主要包括疼痛，特別是外展上臂160-180°時，疼痛明顯、力量減弱[3]；影像學表現(xiàn)上，早期癥狀輕微，很難發(fā)現(xiàn)其病變，隨著疾病進展，核磁可表現(xiàn)肩鎖關節(jié)的水腫信號，再進展X線出現(xiàn)骨質的受累；相關文獻報道，急性創(chuàng)傷后2-4周可逐步出現(xiàn)骨溶解，或是在反復局部應力后的數(shù)月至數(shù)年出現(xiàn)[13]。
鎖骨遠端骨溶解，其臨床癥狀(疼痛、活動受限、肌力減弱)，一般為自限性[3]，數(shù)月內會緩解；針對其治療，選冰敷、吊帶固定、口服陣痛藥及制動等相關治療[5]；其次，可口服消炎陣痛藥及局部注射類固醇激素，可取得有限的療效[10、14]；保守治療無效，可選取手術切除治療，手術切除可以緩解疼痛，不影響肩關節(jié)繼續(xù)活動，但其破壞肩關節(jié)的結構，可能導致肩關節(jié)異?；顒覽15-17]。
創(chuàng)傷后鎖骨遠端骨溶解，發(fā)病率罕見，其病理機制尚未明確，好發(fā)急性外傷人群和反復肩部受力的人群，臨床癥狀一般為自限性，可采取冰敷、制動、激素局部封閉等保守治療，效果不佳可采取手術切除。
